The Registrar’s Office is the sole repository of all student records from the time of admission until graduation. It renders services such as evaluation of academic credits earned, preparation and issuance of transfer credentials, scholastic records, transcript of records, diplomas and certificates of graduation. It further assists the various academic units in preparing the curricula for all academic programs of the institution.
Admission Requirements
1. Junior Learner
• At least 4 years old by June
• NSO Certified Birth certificate (Photocopy)
• Baptismal certificate (Photocopy)
• One (2” x 2”) ID picture
• Alien Certificate of Residency if the student is a foreigner
2. Senior Kindergarten
• At least 5 years old by June
• Baptismal certificate (Photocopy)
• NSO Certified Birth certificate (Photocopy)
• One 2” x 2” ID picture
• Alien Certificate of Residency if the student is a foreigner
3. Grade One
• Must have attended kindergarten and promoted for grade 1
• At least 6 years old by June
• NSO Certified Birth certificate (Photocopy)
• Baptismal certificate (Photocopy)
• One 2” x 2” ID picture
• Report Card
• Alien Certificate of Residency if the student is a foreigner
4. Grade Two - Grade Six and New Pupils
• Report Card
• One 2” x 2” ID picture
• NSO Certified Birth certificate (Photocopy)
• Baptismal Certificate (Photocopy)
• Certificate of Good Moral Character
• With an average grade of 80% in conduct or its equivalent
• Should have obtained a satisfactory rating in the written achievement and mental ability tests administered by the Guidance and Testing Center
• Favorable recommendation from the Principal as a result of the interview.
• Alien Certificate of Residency if the student is a foreigner
5. Grade Seven and New Students
• Interview with the Parents/ Guardian
• Interview with the Student Applicant
• Photo copy of the Report Card reflecting the 1st to 3rd quarter with no failing grades
• Certificate of Good Moral Character duly signed by the Principal
• With an average grade of 80% in conduct or its equivalent
• Photocopy of the Baptismal Certificate & Birth Certificate
• 1 pc 2x2 ID picture
• Photocopy of the Form 137-A or Certification of Subjects taken with corresponding units earned (For transferees only)
• Alien Certificate of Residency if the student is a foreigner
6. For Old Students
• Report card
• Permit to Enroll (incoming Grade 7)
• Students who are recommended for Probation by the Committee on Discipline/Academic may seek re-admission after signing the agreement by parents/guardian and the student.
• Students who will be retained in the same level will not be re-admitted.
• Students who have been recommended for transfer /dismissal due to grave misconduct/incorrigibility and have been subjected to disciplinary action will not be re-admitted.
7. For Grades 11-12
For new students and transferees, they must submit the following:
• Report card (Original)
• Certificate of Good Moral Character
• Photocopy of baptismal and NSO Certificate
• QVR/ESC Certificate for Grade 10 completers from private schools
• Students are not permitted to take any subjects not within the prescribed semester or any advance
subject without prior approval of the Senior High School Coordinator
8.Graduating Students (Grade 12)
a. Application for graduation must be filed in the first term of the last academic year to allow the candidates to be informed of the deficiencies before enrolling for their last term.
b. Graduating students must have completed all academic requirements to qualify as candidates for graduation.
9. New Student/Freshmen (College Level)
· High School Report Card or Form 138
· Original Copy of Birth Certificate from NSO
· Marriage Certificate for Married Students
· Certificate of Good Moral Character
· Four (4) pcs. 2x2 picture

10. Transferees (College Level)

12. Foreign Student
Foreign students are non-Filipino seeking admission to SMCM. They may be admitted after satisfying all admission requirements concerning to observe and recognize the distinctive philosophy, vision, and mission of the school.
All foreign student applicants are required to consult the Registrar’s Office regarding admission and student visa concerns.
Basic foreign student requirements for admission are as follows:
· Student Visa or other authorized visas by the Bureau of Immigration
· Pass the SMCM College Admission Test
· Interview with the Program Head
